چکیده

Introduction Noninvasive ventilation (NIV) represents the standard of care for patients with exacerbation of chronic obstructive pulmonary disease. However, NIV fails in almost 40% of the most severe forms of acute hypercapnic respiratory failure and patients must undergo endotracheal intubation and invasive ventilation. Such transition from NIV to invasive ventilation is associated to increased mortality. Under these circumstances, patients may express a clear intention not to be intubated.

Integrated therapeutic strategy during noninvasive ventilation in a patient with end-stage respiratory disease.

The modern approach (ie, long-term oxygen therapy, rehabilitation, and home mechanical ventilation) has contributed to the survival of patients with chronic respiratory diseases, such as COPD.1 Every day, physicians have to face end-of-life decisions in exacerbations occurring in end-stage patients when endotracheal intubation and/or tracheostomy may be neither appropriate nor accepted (do not ...

Noninvasive pressure support ventilation in COPD patients with postextubation hypercapnic respiratory insufficiency.

Patients with chronic obstructive pulmonary disease (COPD) who have been intubated and mechanically ventilated may prove difficult to wean. Noninvasive ventilation may be used in an attempt to avoid new endotracheal intubation. The efficacy of administration of noninvasive pressure support ventilation was evaluated in 30 COPD patients with postextubation hypercapnic respiratory insufficiency, c...

The successful management of a patient with exacerbation of non-cystic fibrosis bronchiectasis and bilateral fibrothorax using a venovenous extracorporeal carbon dioxide removal system.

Following unsuccessful treatment with noninvasive ventilation (NIV), patients requiring subsequent placement on invasive mechanical ventilation have a high mortality rate. Invasive mechanical ventilation is particularly problematic in patients with acute respiratory failure due to bronchiectasis exacerbation, as it is associated with a mortality rate of 19-35% and prolonged ICU stay. Here, we d...

Another step for noninvasive ventilation in chronic obstructive pulmonary disease patients!

The use of noninvasive positive pressure ventilation (NPPV) in chronic obstructive pulmonary disease (COPD) patients who are not eligible for the technique because of their incapability to spontaneously eliminate accumulated secretions associated with hypercapnic encephalopathy is not recommended and is often considered a contraindication. In a case-control study, an experienced team reported t...

A novel extracorporeal CO(2) removal system: results of a pilot study of hypercapnic respiratory failure in patients with COPD.

BACKGROUND Hypercapnic respiratory failure in patients with COPD frequently requires mechanical ventilatory support. Extracorporeal CO2 removal (ECCO2R) techniques have not been systematically evaluated in these patients. METHODS This is a pilot study of a novel ECCO2R device that utilizes a single venous catheter with high CO2 removal rates at low blood flows. Twenty hypercapnic patients wit...
